Anonymous employee report of discrimination, harassment, and retaliation
This form should be used for the “Anonymous Employee Report of Discrimination, Harassment and Retaliation.”
The college prohibits all forms of discrimination, harassment, and retaliation. A Labouré employee who discriminates against or harasses a member of the Labouré community or assists or encourages another person to engage in such misconduct will be subject to appropriate disciplinary action. Please also refer to the Discrimination, Harassment and Retaliation Policy in the Employee Handbook for more information.
All Labouré College of Healthcare employees are expected to share with the Director of Human Resources any report of discrimination they receive or of which they become aware that involves a Labouré employee. However, employees may also complete this anonymous complaint form. Please note that anonymous reporting may limit the ability for the College to investigate fully and follow-through. Reports of discrimination involving a Labouré student should be directed to the Assistant Vice President of Student Affairs, in accordance with the Non-Discrimination and Harassment Policy for students or complete an anonymous report form below.
